   3Private/Public Limited Company(Sdn Bhd/Bhd) including Single Director Company.To fill up Universal Application Form for BizSmart subscription and Board Resolution.
  • Section D – Registration for Alliance BizSmart Online Banking
  • Section I : Declaration (ALL Mandates required to sign with Full Name, IC Number and Company Stamp)
  • To provide the Board Resolution with complete declaration and Director(s) signature and certified true copy Company Secretary with registered Company Secretary Identification Number.
(All required clauses to be included if customer not using Bank template)

(PLC, assocation, Management, etc)		To fill up Universal Application Form for BizSmart subscription and Board Resolution.
