- Credit Card Application Checklist
For speedy processing, please ensure ALL required documents are submitted with a COMPLETE application form.
If you are an existing principal Credit Cardmember of a locally issued credit card (for more than 12 months):
- Copy of I.C (both sides)
- Copy of Credit Card (front only)
If you do not hold an existing principal Credit Card and are salaried:
- Copy of I.C (both sides)
- Latest 3 month's payslips
- Latest EPF statement and/or Latest 3 months' bank statement where salary is credited
If you do not hold an existing principal Credit Card and are self-employed:
- Copy of I.C (both sides)
- Sole-proprietor/Partnership: Business Registration Form A/B & D
- Sdn Bhd: Form 9, 24 & 49
- Latest 3 months' principal bank account statements
- Latest BE Form (Tax Return Form) with Tax Receipt
- Mail / Fax / Email all the above to:
